  • Thank you for your interest in preparing a School Partner Agreement (SPA) with the School District of Philadelphia.

    This form is intended for programs operating during the 2025–2026 school year (excluding Summer 2026).
  • This form collects the information needed to complete the scope of work for your SPA. Most fields are required, so please take a moment to review the information you’ll need before getting started. Once you’ve gathered the necessary details, completing the form should be straightforward.

    To complete the form, you will need to have the following information on hand:

    • Your organization’s Employer Identification Number (EIN)
    • Contact information for:
      • The person authorized to sign legal agreements on behalf of your organization
      • The primary point of contact for the program
    • Your organization’s mission statement, mailing address, website, and phone number
    • The program name and description, including the main focus area(s) and delivery method
    • The schools where you will operate (you must have confirmation from the principal at each school)
    • The program’s schedule and structure, including start/end dates, days/times of operation, and grade levels served
    • The estimated number of students served
    • The estimated value of the goods/services provided (in dollars)
    • Any student outcomes or District goals your program aligns with
    • (Optional) A logic model or third-party evaluation of your program, if available

    2. Your program will be provided to the listed schools at no cost to the school or the School District of Philadelphia.
